Chronic Care Management
Patients who enroll in our Chronic Care Management (CCM) program receive additional care services from our CCM medical assistant:
- Monthly telephone health review
- After hours availability
- Covered by Medicare and most advantage plans
Preventative &Wellness Visits
Preventative medicine visits are available, including:
- Annual Physicals
- Medicare Annual Wellness Visit
- Age-appropriate cancer screening
Hospital Care & Rehabilitation
Dr. Prieto also serves as attending physician for his patients during hospitalizations at Sebastian River Medical Center:
- Inpatient Care
- Coordination with multiple specialists
